The concept is now 10 years old. Implementation was slow, but major developments have taken place. The surgical part of the procedure can be performed in a totally endoscopic fashion instead of by a mini-incision approach and catheter-based intervention includes the use of drug-eluting stents. Whereas during early development staged approaches were taken, simultaneous interventions have become feasible.
